Give it a name in “Quiz Name”. Select “Survey” in the “ Form Type” as we want to create a survey. Decide a time limit for your survey. A value of “0” indicates no time limit. If you want your users to log in before the poll, check “Yes” in “Require User Login”.

Step 2: Add Questions
